Please make your application as soon as possible.  Make your request online by clicking here.  We will then email you the proper form.  Fill out the form online and e-mail it to tom.goins@okag.org.  Be sure to fill in every space.  Do not send fees.   If you do not have your transcript or photo, submit you application anyway.  We will evaluate your application and respond to you very soon.

Process Overview

PHASE 1: APPLICATION

PHASE 2: SIGNATURE PACKET

•  The Signature Packet with instruction letter are available at Game Days or will be mailed to you. (Return in 5 days.)

PHASE 3: REFERENCE PACKET 

•  The Reference Packet will be mailed to you. (Request References be returned in 10 days.)

PHASE 4: CREDENTIAL EXAM

•  First Saturday of the month at District Office
• Request Synopsis and schedule Exam through deana.smith@okag.org
